Please Help Identifying 2 Ribbons Awarded USS Indy Crew

I had a family member (Frank Romani) go down with the sinking of the USS Indianapolis. I have since been given (handed down) two ribbons and accompanying citations that were awarded to the crew. They are the "Navy Unit Commendation" and the "Combat Action Ribbon". The written description of the Combat Ribbon does not match the colors of the actual ribbon. These items were all sent to a family member from the USS Indianapolis Survivors Organization in a letter dated 9/5/01. I have attached (hopefully) all three documents as well as a photo of the two ribbons.

The USS Indianapolis did receive the combat action ribbon. However, the ribbon to the right of the Navy Unit Commendation Medal is the Navy Presidential Unit Citation (https://en.m.wikipedia.org/wiki/Presidential_Unit_Citation_(United_States))
