Many of you have written asking how to obtain Service Records of relatives to flesh out family history and , in some cases, to learn of their Navy history on the USS Indianapolis or previous ships.


Wanda Little sent the following procedure to learn about relative RM2 Robert A. Rutherford (LAS). Wanda said:

  1. Write to National Personnel Records Center Military Personnel Records 9700 Page Ave. St. Louis MO 63132-5100 Ask them for Form #180 which is a request for records.
  2. Fill out and return. They will send you a NA Form #13075 (National Archives Record Form).
  3. Fill out this form which is more complete than first one.
  4. Mail it back to them. THERE WILL BE COSTS CONNECTED WITH THIS, BUT THEY ARE REASONABLE FOR A REVERED DOCUMENT OF YOUR FAMILY HISTORY.

I recently requested the service record of my Grandfather via the web from the National Archives. It was very easy and just had to print out and fax a signature to get things rolling.

I am trying to locate 2 survivors who were living in Warren, Michigan. I met them at a book review a couple of years ago. The new Mayor of Warren would like to honor them. One of them was 16 when he joined the Navy in the last stages of the war. The Indianapolis was his first assignment.

I have a certificate for the USS Indianapolis's shakedown cruise for Seaman 1st Class, Parker E. Stephenson. 3/16/1933. I'm looking to return the certificate to the family. The closest I can get to the correct Parker Stephenson is one that was born 3/13/1902 and died 2/18/1987. Unfortunately, there's no obit online but I am requesting it from the library where he last lived. Also, there is no listing for him electronically at NARA.
